Accidents
We have the responsibility to prevent accidents and minimize liability through everyday activities. Do not leave students unsupervised. A coach or designated staff member must be present during all practices and open gyms.
When completing use of a facility (gym, field and weight room) the facility must be secured and left the way it was or should have been when the activity was started. This means equipment put away, baskets put up, lights out and doors locked.
If a student becomes injured, contact parents and follow up with the athletic trainer so that all information is present in the accident report.
Under HIPAA (the Health Insurance Portability and Accountability Act) all student injuries are confidential and should only be discussed with parents or personnel directly involved with management of the injury.
The “Insurance Waiver” and “Permission to Treat” forms must be kept in a binder and carried with the team during games (both home and away) in case an accident occurs. Students cannot participate until these forms have been received.
